DESCRIPTION:Our Chronic Disease Self-Management Program is a comprehensive\, evidence-based course tailored for anyone seeking to better manage their chronic conditions. Whether you’re dealing with heart disease\, diabetes\, arthritis\, asthma\, or any other chronic condition\, our program offers practical tools and knowledge to help you take charge of your health.\nOver the course of 6 sessions\, you’ll learn from experienced health educators and connect with peers who understand exactly what you’re going through. Our interactive sessions cover a wide range of essential topics\, including: \n\nTechniques to deal with problems such as frustration\, fatigue\, pain\, and isolation\nAppropriate exercise for maintaining and improving strength\, flexibility\, and endurance\nNutrition advice and how to improve your diet\nEffective communication skills with your family\, friends\, and healthcare team\nHow to evaluate new treatments and understand your medications\n\nIf you refer another participant to the workshop and they attend at least 4/6 sessions\, you will receive a $10.00 referral bonus. \nAlthough it is NOT required for you to commit to all 6 sessions\, it is highly recommended in order to receive the full learning experience.
