SUMMARY:Chronic Disease Self-Management Program (TBPL)

This free\, 6-week virtual workshop is designed to help you take back control of your daily life and break the cycle of pain\, stress\, and fatigue. \nWhat to Expect: This is not a medical lecture. Over 6 weeks\, we will focus on practical\, daily skills to help you feel better: \n\n\nPacing & Energy: Managing fatigue and avoiding the “boom and bust” cycle. \n\n\nSymptom Management: Dealing with pain\, frustration\, and poor sleep. \n\n\nCommunication: Getting the most out of your healthcare appointments and talking to your family. \n\n\nAction Planning: Setting small\, achievable weekly goals to improve your quality of life. \n\n\nThe Perks of Joining: Because this NorWest CHC program is fully funded\, it is 100% free to attend. Plus\, as a thank you for investing in your health\, you will receive: \n\n\nA free copy of the reference book\, “Living a Healthy Life with Chronic Conditions.” \n\n\nA $25 grocery gift card upon completing the 6-week series to help you stock up on healthy essentials! \n\n\nFamily members and caregivers are highly encouraged to register and attend alongside you. \nSecure your spot by filling out the short registration form below.
