First Five Program
The First Five Program is designed for children ages 0-5 who do not have a primary health care provider. Our team includes a Nurse Practitioner, Registered Midwives, Registered Nurse and Lactation Consultant, all offering comprehensive, wraparound services to support your child’s needs.
We can provide the following services:
- Well baby/well child visits
- Immunizations
- Developmental screening
- Nutrition support
- Assistance with navigating the health care system
- Referral requests
- Treatment of non-emergent, episodic concerns such as ear infections, cough, rashes
