Our Care Team is dedicated to improving the health of people in the community by providing a range of services that include providing primary health care, preventing illnesses and promoting health.

The team is made up of a variety of health care professionals that are ready to meet the needs of the community.

Family Physician
  • Provides medical services to clients and members of their family using a broad base of medical knowledge which qualifies them to offer clients comprehensive medical care
  • Fosters a close relationship with the client and family, and provides health care regardless of age, sex, or type of medical problem
Nurse Practitioner
  • Assesses, diagnoses and treats illness and injury
  • Assesses and manages chronic diseases such as diabetes, hypertension and asthma
  • Performs physicals for children, adolescents and adults
  • Orders and interprets tests such as ultrasounds, x-rays and blood work
  • Prescribes medication
  • Provides pre- and post-natal care for pregnant women
  • Provides counselling and treatment to help people with lifestyle changes such as losing weight and quitting smoking
  • Refers clients to other health care professionals and specialists
  • Consults with our on call physician as needed
  • Communicates with your health care team about your social and health care needs
Nurse
  • Takes vital signs and prepares you for the clinical visit
  • Completes immunizations, allergy shots, flu shots and TB testing
  • Ear syringing, suture removal and dressing changes
  • Completes driver’s physicals, prenatal examinations, well woman and well child examinations
  • Directs you to the appropriate care provider at the right time
  • Responds to your telephone requests and communicating with your health care providers about your needs
  • Supports you to manage your health conditions through health teaching
Therapist
  • Offers support to assist with addressing various mental health issues, including depression, anxiety, grief and loss, relationship issues, separation/divorce, trauma, and addiction
  • Provides various models of practice such as mindfulness based stress reduction, emotionally focused therapy, Cognitive Behavioural Therapy and
  • Harm reduction
  • Offers psychoeducational workshops on a variety of topics
Telemedicine Nurse
  • Registered Nurse
  • Provides healthcare services through video, phone, email and messaging
  • Provides high-quality care to all clients
Foot Care Nurse
  • Can help reduce heavy calluses, corns and trim deformed or thickened nails
  • Assesses feet for circulation, sensation, edema (swelling), and assess for the potential risk of ulcer development and falls
Community Health Worker
  • Provides information and assistance to access community and health resources in areas such as income, housing, food security, health literacy, life skills, health, mental health and social supports
  • Provides information about our primary health care, health promotion services, and other community-based services
  • Encourages clients to participate in life skills and financial literacy programs, either individually or in a group setting
  • Encourages clients to participate in health and wellness programs, illness prevention, and self-management programs
  • Provides coaching to help clients achieve their personal goals
  • Offers crisis response services when needed
  • Assists in securing personal identification and provides support with forms
Registered Dietitian
  • Helps develop practical solutions for individuals, families, and communities
  • Provides tailored advice in regards to a client’s needs and access to food
Harm Reduction Worker
  • Actively engages with members on and offsite
  • Establishes trust with clients 
  • Provides harm reduction supplies and education
  • Connecting clients with relevant services
Midwife
  • Comprehensive, client centered sexual and reproductive healthcare
  • Provides health promotion and health education
