Homelessness & Addiction Recovery Treatment Hub (HART Hub)
What is a HART Hub?
The Homelessness & Addiction Recovery Treatment (HART) Hub is one of 28 newly established HART Hubs across Ontario, created to provide coordinated, compassionate support for individuals experiencing homelessness and substance-related challenges. Through a collaborative, community-based approach, the Hub delivers integrated services that address the complex factors affecting health and well-being, including housing support, primary health care, addiction treatment, mental health services, social supports, cultural programming, and long-term reintegration assistance. By bringing these services together in one accessible network, the HART Hub helps individuals build stability, strengthen connections, and move toward lasting recovery and improved quality of life.
