Chronic Disease Self-Management Program (TBPL)

Living with an ongoing health condition—like arthritis, heart disease, chronic fatigue, or fibromyalgia—can be exhausting. This free, 6-week virtual workshop is designed to help you take back control of your daily life and break the cycle of pain, stress, and fatigue.

What to Expect: This is not a medical lecture. It is a highly interactive, peer-led workshop where you will connect with others who truly understand what you are going through. Over 6 weeks, we will focus on practical, daily skills to help you feel better:

  • Pacing & Energy: Managing fatigue and avoiding the “boom and bust” cycle.

  • Symptom Management: Dealing with pain, frustration, and poor sleep.

  • Communication: Getting the most out of your healthcare appointments and talking to your family.

  • Action Planning: Setting small, achievable weekly goals to improve your quality of life.
